As an injured worker or person involved in a motor vehicle accident, completing this Consent form nominates HEALTH ABILITY as your rehabilitation provider so we can support, guide, and assist you throughout your recovery in collaboration with your doctor.

 Nomination & Consent

I consent to nominate HEALTH ABILITY as my chosen rehabilitation provider to support and assist me throughout my recovery.

By providing this consent, I authorise HEALTH ABILITY to communicate with my insurer, treating practitioners, and any relevant parties involved in my injury/illness/condition management.

I understand that this nomination enables HEALTH ABILITY to coordinate rehabilitation services on my behalf and to receive and share information necessary for planning, delivering, and monitoring my recovery.

I also understand this information will be respected privately and confidentially in line with the Australian Privacy Principles and I can request a copy of the Privacy Policy and understand I can change or cancel my consent.

 RC's Role in Helping You

Rehab Consultants (RC) are allied health professionals who are trained and accredited to provide you expert assistance and support to help you recover from your injury, illness, or condition.
Rehab Consultants have tertiary qualifications, experience, and experience to provide advice, assessment and intervention to help you recover.
They also support you in communicating with the key stakeholders (for example your doctor, physio, employer/supervisor, case manager, etc). involved in your claim.
If HEALTH ABILITY is unable to assist you, you can subsequently select an alternate rehabilitation provider.

Our team consist of allied health professionals including Physiotherapists, Exercise Physiologist (EP), Occupational Therapist (OT), Psychologists, and Rehabilitation Counsellors.
